Attention Parents/Guardians of students interested in HMS Athletics

Hello everyone.
 
On May 1, Harris Middle School officially launched FinalForms. What is FinalForms? It is an online database that keeps up with all required medical information and athletic paperwork for parents, students, and coaches. In years past, we required all of this information but in paper-form. We are essentially trying to move our athletic department and school out of the filing cabinets and into the future.
 
FinalForms will make it easier for coaches and the athletic department to review and learn all of the important medical information about our students to keep them safe. Signing up for FinalForms is 100% Mandatory. If a student is going to play sports at Harris, a parent/guardian and that student must be on file with FinalForms.
 
Signing up is free. However, if your child makes one of our teams, there will be a $3 dollar service charge paid at their first team meeting. This fee will cover multiple sports the student may play and last for the entire school year.
 
Signing up is not complicated. I have developed a step-by-step guide for parents and for students to sign up. I have translated it into Spanish as well. I encourage every parent and every child interested in playing sports to immediately go to FinalForms.com, sign up, and begin to complete the forms for their student.
 
This will make it easier to place the students onto any teams that they make for the school year. For any family who has trouble with the process, I will personally be at every team meeting this year with laptops to help finish the process. But again, do not wait until then. Everyone try to get started. Good luck everyone.
