Dear Parent/Guardian:
We must have a current immunization certificate on file for your child.
Students entering 7th Grade are required to have a TDAP booster
Unless your child has a medical exemption or a religious exemption, he/she must receive the necessary immunizations before September 6, 2022. We will need the official State of Tennessee CERTIFICATE OF IMMUNIZATION for school records. This form must be signed or stamped by the healthcare provider.
Once your child has received the necessary immunizations, the State of Tennessee CERTIFICATE OF IMMUNIZATION will be provided by the clinic that administered them. Please bring this form to school before September 6, 2022.
Your child must have these immunizations by September 6, 2022, or he/she will NOT be allowed to return to school until they are in compliance with the state law. If your child has a religious or medical exemption, please contact me. In the event that your child is unable to get an appointment with their physician prior to the September 6, 2022 deadline, please bring an appointment card from the physician. If you have any other questions or concerns, please contact me at the school.

A menos que su hijo(a) tenga una exención médica o una exención religiosa, debe recibir las vacunas necesarias antes del 6 de septiembre de 2022. Necesitaremos el CERTIFICADO OFICIAL DE INMUNIZACIÓN DEL ESTADO DE Tennessee para los registros escolares. Este formulario debe estar firmado o sellado por el proveedor de atención médica.
Una vez que su hijo(a) haya recibido las vacunas necesarias, la clínica que las administró le proporcionará el CERTIFICADO DE INMUNIZACIÓN del Estado de Tennessee. Por favor, traiga este formulario a la escuela antes del 6 de septiembre de 2022.
